LEAP training required formS

All teen LEAP participants must complete and submit this teen form.

Likewise, one parent/guardian must complete the parent form.

While filling out the forms, if you have additional questions, please reach out to the Sponsor of your training, or discuss those with the team member who facilitates each of your 1-hour pre-training support call.

Each form can take anywhere from 15 - 20 minutes depending on the person and the desired length of your responses. You can opt to SAVE THE FORM AS YOU GO and have it sent to your email. Or, you can fill it out in one time.
